Is there a form used to protest an award for permanent impairment? I downloaded the document submission form. Is there another form or do I just write a letter protesting the decision? |

You may use the document submission form to protest. Just be sure to indicate the date of the Division's Order that you are protesting and give a brief description of the Order in the area provided. Example: Division's Order date 5/15/03 granting 5% PPD. You can also submit a letter protesting the decision with, or without, the document submission form. We prefer that you do use the document submission form'. Example of letter protesting award: "I protest the Division's Order dated 5/15/03 granting a 5% PPD award." On all communications, be sure to correctly list the claim number. |
