Rate & Form Filing Information
Public Information Kiosk
Located in room 301 of the West Virginia Offices of the Insurance Commissioner, 1124 Smith Street, Charleston, WV. All public rate & form filings are available for viewing through the kiosk. The kiosk allows access to filings stored in the agency's imaging system as well filings stored with the National Association of Insurance Commissioners System for Electronic Rate & Form Filing (SERFF). Due to the limited availability of access via this single terminal, reservations are recommended and may be made up to 30 days in advance by calling (304)-558-2094.
No fee is associated with the use of the kiosk. However, copies of filings viewed at the kiosk may be requested and obtained for the appropriate fee.
